The intention of this “report” is to make a quick, simple, way for a peer or service head to make a place card for a future nomination to the EMS Annual Awards. Far too many time great accomplishments and/or outstanding responses go unacknowledged, not because they aren’t appreciated, but because the necessary nomination was not submitted in by the deadline. Once this report is submitted – the EMS Bureau Staff member assigned to monitor this information will contact the person who submitted the form during the annual nomination window. That individual will be reminded of the past event and be asked to submit a full nomination, if they feel it appropriate.
